2/1/2023 EnchantaVaughn Approved 1. Behavior: Client hugged QMHP upon arrival. Client helped QMHP set up a quiet area in the house for the session to begin. Client appeared to be enjoying her day so far. Client was respectful and careful not to wake her little brother up who was sleeping on the couch. Client was happy to see her mom smiling and in such a good mood, which immediately put her in a better mood. Client reassured she understood the rules and expectations and did not need to review her treatment plan any further. 2. Behavior: Client exhibited appropriate social skills and team work during psycho educational activity. Client displayed a good attitude and was very respectful in the session, not interrupting counselor or being disruptive when it wasnât her turn to talk. Client was given positive feedback and engaged in self-reflection. Client was receptive to feedback and verbalized good thoughts during the session. Client was supported for displaying active listening skills and attending to her work. 1. Intervention: QMHP provided individual and family counseling based on specific treatment objectives. QMHP reminded client of goals and objectives. QMHP collaborated with mom in regard to new home rules and regulations, behavior management, effective teaching practices, following verbal and non-verbal instruction, and an appropriate incentive system. QMHP observed client complying with home rules and expectations. QMHP helped client to see the positives and to take advantage of receiving additional help at home. 2. Intervention: QMHP praised client for utilizing acquired skills needed to continue following her ISP. QMHP processed client positive responses and challenged client to maintain appropriate behaviors throughout the entire week. QMHP encouraged client to resist negative behaviors in efforts to refrain from requiring to be redirected by her mom so many times. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. 1. Response: Client displayed management of individualized behaviors. The client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation with parent, and waiting for her turn to talk without cutting off others to get more attention. 2. Response: Client said she loves coloring activities/art and utilizes this to relax and stay focused. Client has become more involved in age appropriate social activities while being supportive of others. Client did not display any outbursts during the role play exercise. Client demonstrated expectations and responded well to redirection from mother and VACCD staff. Client learned to look for similarities rather than differences in the way she acts versus how other ADHD children act, and the benefits of this was reviewed in the session.
